Forms

Trainees need to fill out the following forms in order to avail ETP funded training. Students need to fill out and fax the following forms prior to the first day of the class. Also, the original signed 100E must be mailed to Saisoft. See the left panel for Saisoft contact information.

If you would like to scan and email your forms as PDF documents, please email them to forms@saisoft.net.

Company Form:

1. Certification Statement (100E) Form. (This form is not downloadable. It will be emailed to you)

2. IT Training Policy (Training Policy Form) 

3. Trainee Record. (Form ETP 104)  

4. Trainee Authorization For On-line enrollment (Form ETP 104 AUTH)

5. Copy of the most recent pay stub. This is required to confirm your employer, to ensure you make over $14.02 per hour ($28,040 per year) and that you pay California taxes.

6. Depending on the course the student is taking, fill out ONE of the following

    - Windows Server Track
   -  VB/C#/ASP.NET Track
   -  Security Track
   -  Linux Track
   -  MySQL Track

   -  Cognos Track

Procedure to make-up missed class

a. Log into http://portal.saisoft.net and navigate to the web site for the class you are enrolled in.

b. Download the lessons (PDFs, Powerpoint slides etc.) and any corresponding lab exercises assigned by your teacher and complete the lab exercises.

c. Attend class sessions for the number of hours you have missed. You have the flexibility to choose from any of the currently running or upcoming courses at http://www.saisoft.net/schedule.html. Please select the class you'd like to attend and email makeup@saisoft.net to request a login ID for the class of your choice. Note that you may not find the exact class you missed, however, you will have quite a few to choose from at any given point of time.
